Observed dirt, grime, and / or food particle buildup on floor and floor sink on right side of the bar ice machine. Improve cleaning frequency and methods to ensure all equipment is kept in an appropriate state of cleanliness; ensure all equipment surfaces are smooth and easily cleanable, and all potential food contact surfaces are sanitized on a regular basis.

Observed the floors under the bar sinks are peeking up and need cleaning. Evaluate, repair and deep clean.

A food facility shall immediately cease operation when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall remain closed until:
(a) there is no longer evidence of a vermin infestation.
(b) all contaminated surfaces have been cleaned and sanitized.
(c) contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.

All contributing factors must be resolved as quickly as possible. If there is a vermin infestation that warrants a major violation, then the permit to operate may be suspended. Professional pest control may also be required as frequently as needed to prevent the infestation from recurring.
Observed rodent droppings in the dry storage room and in closet with fuse box.

Observed an alive cockroach and some dead cockroaches caught inside a trap in the dry storage room.

Continue your pest service. Organize dry storage room to prevent harboring sites. The premises of each food facility shall be kept free of vermin.

Observed the cutting board at salad prep refrigerator is grooved and discolored, does not appear to come completely clean, and no longer constitutes a smooth and easily cleanable surface. Repair or replace damaged and discolored cutting boards; ensure that all food / utensil contact surfaces are smooth, easily cleanable, and nonporous.

Observed left door of the salad prep refrigerator has a torn door seal. Repair.

Observed the ceiling panel of the walk in freezer is falling down leaving a gap that exposes the wood and interior of the panel. Repair.

Observed a large gap around the drain pipe of the three compartment sink that appears to lead outside. Seal.

Observed day light visible around the two rear doors. Seal.

Sealing both will help prevent entrance of vermin.

Observed a crack in the wall between the FRP and dry wall directly above the dish room hand sink. Repair.
