Observed sharing table in use at this location that includes whole uncut fruit, commercially prepackaged baggies of peeled baby carrots that state 'keep refrigerated' on the labels, and half pint cartons of milk. When asked about the disposition of sharing table items at the end of the day, staff reported items are returned to the kitchen and coolers.

Cease this practice. Any PHF (potentially hazardous food items) that have been placed on the sharing table areas must be served or discarded within 4 hours of being removed from temperature control. Do not reuse or reserve PHF items from day to day. Discussed possible strategies to reduce waste (offer versus serve, second snacks during afternoon recess, etc). Lunch period serving time began at 10:45 AM per staff. Milk and baby carrots from today's sharing table to be served or discarded by 2:45 PM today. Ensure all staff are correctly trained for this procedure. Corrected during inspection.

2.00 Post permit in a conspicuous place (It is recommended that the permit be in customer view.). Post a notice advising patrons that a copy of the most recent routine inspection report is available for review by interested parties. Keep a copy of the most recent health inspection report at the facility for customer review purposes.

Post and maintain a legible and conspicuous sign in each toilet room directing attention to the need to thoroughly wash hands after using the toilet.

The non-food-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Equipment shall be reassembled so that food-contact surfaces are not contaminated.
Observed decoration on the hood in the main kitchen. Remove to prevent an accumulation of dust and debris.

Observed the use of raw wood pellets in the walk-in freezer. Wood does not constitute a smooth, easily cleanable, and non absorbent surface. Replace or provide.

A food facility shall not be operating if there is gross contamination of food contact surfaces that may result in the contamination of food products. Observed the sanitizer from the mixer was measured at 150 ppm of quaternary ammonia.
