1) 1ST RESTROOM: The flooring from the previous inspection showed splitting and curling in several areas. The flooring is now cut in several areas to prevent the curling. This makes the flooring no longer smooth, easily cleanable, and nonabsorbent. Replace the flooring with the proper coving (Rubber topset is no longer approved as a new installation). Provide samples of the proposed flooring and coving materials to our office for approval prior to installation. Installation of unapproved flooring will be required to be removed at the operator's expense.

2) 2ND RESTROOM: Remove all the stains and graffiti from the walls and FRP walls.

3) 2ND RESTROOM: Replace the missing electrical outlet and light switch covers.

Lockers or other suitable facilities shall be provided and used for the orderly storage of employee clothing and other possessions. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens, and single-use articles cannot occur. Dressing rooms or dressing areas shall be provided and used by employees if the employees regularly change their clothes in the facility.
1) MOP SINK: 3 used mops left inside mop sink. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.

2) RETAIL SALES FLOOR: Multiple boxes of products within the aisles during business hours awaiting to be stocked on the shelves by employees. Discontinue this practice as it creates potential trip hazards throughout the store while waiting for the employees to stock the products. Try stocking products during non-business hours or only bring out small quantities of products to stock each time.

3) BACK STORAGE/INVENTORY: Area has too many products and not enough storage space. Non-food products are mixed with food products. Boxes are stacked high above the wheeled cart heights. Boxes were observed falling off near employee while employee was trying to provide access to the walk-in refrigerator. Access to both the walk-in refrigerator and freezer were blocked by wheeled carts and boxes. Area is full of clutter and has approximately 3 foot wide walking space only, creating potential sources of injuries. Organize area and use shelves. Discontinue ordering too many products as there is not enough storage space in this facility.

(a) At least 10-foot candles for the following:
(1) At a distance of 30 inches above the floor, in walk-in refrigeration units, and dry food storage areas.
(2) At a working surface on which alcoholic beverages are prepared or where utensils used in the preparation or service of alcoholic beverages are cleaned.
(3) Inside equipment, such as reach-in or under-the-counter refrigerators.
