DENTED FOOD CANS: Two 20 oz Dole Pineapple Chunks cans and three14 oz Wolf Brand Chili Hot Dog Sauce cans were severely dented/damaged on the sides and toward the top areas. Approved base coving shall be provided in all areas, except customer service areas and where food is stored in original unopened containers. Food facilities shall be fully enclosed. All food facilities shall be kept clean and in good repair. Toilet facilities shall be provided for patrons: in establishments with more than 20,000 sq ft.; establishments offering on-site food consumption. (113953, 113953.1, 113953.2, 114250, 114250.1, 114276) 1.00 Provide clean toilet facilities in good repair. Provide permanently installed toilet tissue dispenser(s) in all toilet facilities. 2) WOMEN'S RESTROOM: Heavy dust accumulation on the square ventilation cover in the ceiling. Clean.

3) WOMEN'S RESTROOM: A roll of household paper towels was sitting on top of the black paper towel dispenser. Provide paper towels inside the black wall dispenser.

4) MEN'S RESTROOM: The sheet vinyl floor base is pulling away from the wall in several areas. Repair.

5) MEN'S RESTROOM: Patch the hole in the ceiling and repair the cracked ceiling paint areas.

6) MEN'S RESTROOM: Heavy dust accumulation on the square ventilation cover in the ceiling. Clean.

7) MEN'S RESTROOM: Cracked/damaged sheet vinyl floor arear near the toilet stall. Repair.
